Located just off St. George Boulevard in the heart of downtown. ## What to bring When you come in for your appointment, bring a few things to make the process fast: - **Proof of income:** Your last two paystubs, an awards letter for fixed income, or other proof of what you earn. - **A valid government-issued ID:** A driver's license, state ID, or U.S. passport. - **Your Social Security card.** - **Proof of residence:** Something dated within the last 45 days showing your current Utah address, like a utility bill or lease.
